=== Nitriles in medicine === Nitriles occur in numerous classes of drugs. Between 2010 and 2020, at least one drug containing a nitrile function was approved annually by the US Food and Drug Administration. The nitrile group exhibits characteristic physicochemical properties that are important in drug design. Structurally, it has a linear geometry and occupies very little space—approximately one eighth of the volume of a methyl group. As a ligand substituent, it is therefore well suited to occupying narrow and deep cavities within the binding site of a target protein that are otherwise difficult to access. Incorporation of a nitrile group into a molecule generally reduces its octanol-water partition coefficient or increases its aqueous solubility. This often favorably influences bioavailability, plasma half-life, and thus the duration of action of lipophilic compounds. In medicinal products, the nitrile group is typically metabolically stable. The nitrile group is isosteric with the carbonyl group, the hydroxy group, and the chlorine atom. This has been particularly problematic for Mauritius, as it has had numerous social effects for the population. Facilitators in Mauritius, and other trans-shipment states, are often paid with product which means they sell it to locals. Therefore, consumer markets develop locally, thus increasing demand and drug-dependency. Drug abuse is a significant problem within Mauritius, which is regularly evidenced by the ‘World Drug Report’. In 2008, the report illustrated that 2% of the population of Mauritius were affected by the use of opioids, such as heroin. A significant proportion of these drugs enter the country through the coastline, which has been linked to a lack of surveillance in these places and the surrounding maritime areas. Many of the illicit substances enter on private yachts, which are not regularly checked. In 2012, a steward previously involved in drug trafficking, expressed that the conditions of the maritime environment made smuggling simple as the oceans are so vast and avoiding capture is easy. A 1986 report from the CIA expressed concern over the entry of illegal drugs to Mauritius, stating that these were both consumed locally and smuggled onwards to South Africa. Therefore, this has been a long-standing problem, which has become entrenched in Mauritian society. == History == Paniz-Mondolfi was born in Caracas, Venezuela in 1976. His mother is a pediatrician and his father an architect. His grandfather, Edgardo Mondolfi Otero, a biologist and zoologist. Two of his uncles were physicians with special ambitions in the field of sciences and research. His childhood was shared between Caracas and Kenya. Paniz-Mondolfi has a master's degree in parasitology and tropical diseases; he did international fellowships in microbiology, molecular genetics, and skin disease. He also did a second medical residency in the United States in pathology. He isolated and described a new species of parasite that had infected a NY resident, and a new mycobacterium that sickened two people in Connecticut. He earned an MD and PhD and studied under Jacinto Convit the leprosy researcher. He was a pathologist at the IDB Biomedical Research Institute in Barquisimeto, Venezuela until 2019 when he fled to the United States. He is the founder of the Venezuelan Science Incubator, an independent health research organization. Paniz-Mondolfi is assistant professor of pathology, molecular and cell-based medicine at the Mount Sinai Icahn School of Medicine. He is also affiliated with the Yale Cancer Center. In 2020 he focused his research on the COVID-19 pandemic and the effects the virus had on minority children. Official Chinese resistance to opium was renewed on September 20, 1906, with an antiopium initiative intended to eliminate the drug problem within 10 years. The program relied on the turning of public sentiment against opium, with mass meetings at which opium paraphernalia were publicly burned, as well as coercive legal action and the granting of police powers to organizations such as the Fujian Anti-Opium Society. Smokers were required to register for licenses for gradually reducing rations of the drug. Action against opium farmers centered upon a highly repressive incarnation of law enforcement in which rural populations had their property destroyed, their land confiscated and/or were publicly tortured, humiliated and executed. Addicts sometimes turned to missionaries for treatment for their addiction, though many associated these foreigners with the drug trade. The program was counted as a substantial success, with a cessation of direct British opium exports to China (but not Hong Kong) and most provinces declared free of opium production. Nonetheless, the success of the program was only temporary, with opium use rapidly increasing during the disorder following the death of Yuan Shikai in 1916. Opium farming also increased, peaking in 1930 when the League of Nations singled China out as the primary source of illicit opium in East and Southeast Asia. Many local powerholders facilitated the trade during this period to finance conflicts over territory and political campaigns.
